چکیده
phylaxis. Conspicuous by its absence is any report of such devices precipitating an embolic event during general surgery. We cannot establish causality. It is possible that the embolism would have occurred irrespective of the placement of the sequential devices because of the presence of multiple risk factors in our patient, including age, obesity, malignancy, estrogen replacement hormones, and relative immobility (because of illness). Nevertheless, the temporal association between activation of the pneumatic device and sudden onset of symptoms at least suggests that the application of preventive measure may have paradoxically triggered a life-threatening complication. Such an anecdotal report should not be held up as the sole basis for changing clinical practice, particularly when benefits of these compression devices outweigh the risk of complication. However, we suggest that these well-intentioned prophylactic measures may have adverse consequences, particularly if they are not used appropriately. Clinicians who care for patients with moderate to high risk for DVT who are fitted with these devices should remain alert for signs and symptoms of acute embolism, especially during the first few minutes and hours after their application.

Oxygen Affinity of Hemoglobin in Persons with Acute Myocardial Infarction
THE OXYGEN affinity of hemoglobin (Bohr effect)1 may vary in different subjects. Under conditions of organ ischemia, these variations may be of significance. A study was undertaken to assess the affinity of hemoglobin for oxygen among patients in various states. Included in the study were patients with typical acute myocardial infarction, subjects with clinical and electrocardiographic signs of...

متن کاملFactors affecting perioperative transfusion decisions in patients with coronary artery disease undergoing coronary artery bypass surgery.
BACKGROUND A high proportion of patients having cardiac bypass surgery receive erythrocyte transfusions. Decisions about when to transfuse patients having surgery for coronary artery disease may impact on erythrocyte utilization and patient morbidity and mortality. There are no published data about the factors that influence physicians' decisions to transfuse erythrocytes to these patients. The...
متن کاملA polymerized bovine hemoglobin oxygen carrier preserves regional myocardial function and reduces infarct size after acute myocardial ischemia.
The purpose of this study was to test if HBOC-201, a hemoglobin-based oxygen-carrying solution, can decrease infarct size (or Inf) during acute, severe myocardial ischemia and reperfusion. To test the impact of HBOC-201 on infarct size, ischemia was produced in 18 dogs by coronary stenosis to achieve 80-95% flow reduction for 195 min along with pacing 10% above the spontaneous heart rate, follo...
متن کاملPreservation of canine myocardial high-energy phosphates during low-flow ischemia with modification of hemoglobin-oxygen affinity.
Conventional approaches for the treatment of myocardial ischemia increase coronary blood flow or reduce myocardial demand. To determine whether a rightward shift in the hemoglobin-oxygen saturation curve would reduce the metabolic and contractile effects of a myocardial oxygen-supply imbalance, we studied the impact of a potent synthetic allosteric modifier of hemoglobin-oxygen affinity, a 2-[4...
